The prostate is a small gland located just below the bladder, surrounding the urethra. It plays a key role in reproductive and urinary function — and changes in prostate health can significantly affect quality of life. Symptoms such as urinary urgency, pelvic pain, incomplete emptying, sexual dysfunction, and post-ejaculatory discomfort are commonly linked to both inflammatory and mechanical conditions involving the prostate. At the Performance Medicine Institute, we provide a comprehensive, whole-person approach to prostate-related concerns that goes beyond imaging and PSA levels alone.
We combine musculoskeletal evaluation, pelvic floor assessment, hormonal testing, urologic collaboration, and precision therapies — including pelvic floor rehabilitation, manual therapy, laser therapy, and lifestyle optimization — to help patients regain comfort, function, and peace of mind.
We treat a variety of prostate-related conditions, including:
Chronic Prostatitis / Chronic Pelvic Pain Syndrome (CP/CPPS)
This is the most common prostate-related condition in men under 50. Symptoms include pelvic pressure, deep perineal aching, painful urination or ejaculation, and increased urinary frequency. Our treatment targets pelvic floor tension, pudendal neuralgia, inflammation, and autonomic dysregulation using a multimodal approach that includes manual therapy, downtraining, next generation sequencing to screen for potential infections, behavioral strategies, medication management, and laser therapy for pain and inflammation.
Benign Prostatic Hyperplasia (BPH)
BPH is a non-cancerous enlargement of the prostate that can compress the urethra, causing weak stream, urinary retention, and nighttime urination. We work with patients to partially or totally alleviate the symptoms of BPH, restoring normal bladder function, and preventing frequent trips to the bathroom at night. In some cases surgery is necessary to treat BPH, and we work closely with urologists in the post-operative rehabilitation of patients who have undergone various BPH surgeries. For these patients, bladder function is much more rapidly restored, and erectile function returns to normal. The central components to our treatment program are pelvic muscle retraining, high frequency neuromuscular activation, bladder control strategies, and adjunctive therapies to improve flow dynamics and reduce dependency on medications.
Post-Prostatectomy Pelvic Dysfunction
After prostate surgery (e.g., for cancer), patients may experience urinary incontinence, erectile dysfunction, and pelvic floor deconditioning. We deliver structured post-operative rehab focused on continence recovery, sexual function, and pelvic stability — using biofeedback, strength training, and regenerative modalities including laser therapy.
Prostate-Associated Ejaculatory Pain
Pain after ejaculation may be linked to prostatitis, pelvic floor tension, or nerve irritation.
We assess for muscular hypertonicity and neural entrapment and provide soft tissue release, pelvic floor downtraining, and neuromodulation techniques to reduce post-orgasmic discomfort.
Prostate-Related Erectile or Libido Dysfunction
Chronic inflammation, medical therapy, or surgical intervention can contribute to impaired arousal or erectile function. We evaluate hormonal status, pelvic mechanics, and neurovascular health, offering a plan that may include hormone optimization, pelvic floor therapy, and regenerative support strategies.
