Scrotal health is often overlooked but plays an important role in comfort, fertility, hormone regulation, and sexual function. Pain, swelling, or anatomical changes in the scrotum may be caused by trauma, infection, vascular congestion, or mechanical irritation — and can be both physically and psychologically distressing. At the Performance Medicine Institute, we provide thorough, discreet evaluation and treatment of scrotal conditions using a multidisciplinary approach that includes physical exam, scrotal ultrasound, hormonal assessment, pelvic floor evaluation, and advanced therapeutic options including regenerative injections and laser therapy.

We care for the following scrotal conditions:

Varicoceles and Hydroceles

A varicocele is an enlargement of the pampiniform venous plexus, often described as a “bag of worms” appearance on exam. A hydrocele is a fluid-filled sac surrounding the testicle, typically causing painless swelling. These conditions can lead to a dull ache, scrotal heaviness, and impaired sperm production. Our therapeutic approaches help to treat pain associated with varicoceles and hydroceles, and also address pelvic congestion and muscular compensation patterns to improve venous outflow and reduce the size and symptoms of these conditions.

Epididymitis / Orchitis

Inflammation of the epididymis or testicle may present with pain, swelling, warmth, and urinary symptoms. Causes include bacterial infection, STI, or post-vasectomy inflammation.

We offer rapid diagnosis, antibiotic or anti-inflammatory treatment, STI screening, and supportive care including scrotal support and pelvic floor therapy for chronic cases.

Scrotal Pain Syndrome / Post-Vasectomy Pain

Chronic or intermittent scrotal pain may develop after trauma, surgery, or spontaneously. Sometimes referred to as Chronic Scrotal Content Pain (CSCP), this condition can be quite painful. Neuropathic pain or pelvic floor dysfunction are often contributing factors. We address underlying nerve entrapments, muscular hypertonicity, and inflammation using pelvic rehabilitation, neuromodulatory strategies, laser therapy, and trigger point release. Many men experience very rapid pain relief.

Testicular Torsion (Emergency Referral)

Torsion involves twisting of the spermatic cord, cutting off blood flow to the testicle — a surgical emergency. Some patients who undergo surgery to treat the torsion will develop chronic pain and erectile dysfunction. Our treatment approach comfortably reduces pain and improves function.

Scrotal Neuropathy

Scrotal neuropathy, or pain in the scrotum due to nerve damage, can be caused by various conditions, including nerve entrapment, diabetes, infections, trauma, hernias, or kidney stones. In some cases, the ilioinguinal nerve and genitofemoral nerve, which supply the scrotum, can become compressed or irritated, leading to pain. We treat neuropathy by identifying causal factors, and use pelvic floor therapy, lasers, myofascial release, and other techniques to eliminate pain and restore function.
