Hangovers: Why They Happen, What Helps, and How to Recover Safely

What is a Hangover?

A hangover is a collection of symptoms that can occur after alcohol intake, usually as blood alcohol levels fall toward zero. Symptoms may include headache, nausea, fatigue, thirst, dizziness, light sensitivity, sound sensitivity, poor concentration, stomach upset, irritability, anxiety, low mood, sweating, shakiness, and reduced exercise performance.

A hangover is not caused by one single factor. It is the combined effect of alcohol metabolism, acetaldehyde exposure, dehydration, electrolyte shifts, sleep disruption, stomach irritation, inflammation, oxidative stress, blood sugar changes, nervous system effects, and other compounds found in some alcoholic drinks.

Why Hangovers Happen

Alcohol affects multiple body systems at once. It changes fluid balance, irritates the stomach, disrupts sleep architecture, alters blood sugar regulation, increases inflammatory signaling, and places metabolic demand on the liver.

This is why simply drinking water does not always prevent a hangover. Hydration helps, but a hangover is not just dehydration. Poor sleep, acetaldehyde exposure, inflammation, stomach irritation, and total alcohol dose all contribute.

Alcohol is metabolized primarily in the liver. Ethanol is converted into acetaldehyde, then acetaldehyde is converted into acetate. Acetaldehyde is a toxic intermediate that can contribute to flushing, nausea, headache, inflammation, and feeling unwell.

The body can only clear alcohol at a limited rate. Coffee, cold showers, exercise, or IV fluids do not make the liver rapidly metabolize alcohol. Time is the main factor that lowers blood alcohol levels.

Alcohol suppresses vasopressin, also called antidiuretic hormone. This causes the kidneys to produce more urine, which can contribute to dehydration, thirst, headache, dizziness, dry mouth, and fatigue.

Vomiting, diarrhea, sweating, and poor intake can make fluid and electrolyte loss worse. Electrolytes such as sodium, potassium, and magnesium may help some people feel better when dehydration or poor oral intake are part of the problem.

Hydration is useful, but it does not erase the effects of heavy alcohol intake. It supports recovery rather than curing the hangover.

Side Effects of Excess Alcohol Consumption

Hangovers are associated with inflammatory and oxidative stress responses. This may help explain why people feel achy, mentally foggy, light-sensitive, irritable, and generally unwell.

Alcohol can make someone feel sleepy, but it usually worsens sleep quality. It can fragment sleep, reduce restorative sleep, disrupt REM sleep, worsen snoring, worsen sleep apnea, and cause early morning wakefulness. Poor sleep is one of the major reasons people feel exhausted, foggy, anxious, and physically under-recovered after drinking. For athletes and active adults, alcohol-related sleep disruption can also impair training adaptation, reaction time, coordination, and muscle recovery.

The brain is especially sensitive to poor sleep, dehydration, blood sugar swings, and inflammatory signaling. This is why a hangover may feel like more than a headache. It can feel like low motivation, reduced focus, poor mood, and slower mental processing.

Alcohol can irritate the stomach lining, increase acid production, worsen reflux, and alter stomach emptying. This can lead to nausea, vomiting, abdominal discomfort, diarrhea, poor appetite, and burning or sour stomach symptoms.

"Pregaming" or Prevention: What Actually Helps

The most reliable prevention strategy is drinking less. Practical steps include setting a drink limit before starting, drinking slowly, eating before and during drinking, alternating alcoholic drinks with water or electrolyte drinks, avoiding shots or rapid drinking, and stopping earlier than usual.

Avoid drinking close to bedtime when possible. Alcohol late at night is especially disruptive to sleep and recovery.

Avoid mixing alcohol with sedatives, opioids, benzodiazepines, sleep medications, muscle relaxers, or other substances that depress breathing or alertness. This combination can be dangerous.

Plan transportation before drinking. Alcohol impairs judgment before a person may realize they are impaired.

Several supplements have been marketed for reducing hangovers, but many lack scientific support. There are some supplements with scientific support, such as ZBiotics Pre-Alcohol, which is a genetically engineered probiotic product designed to break down acetaldehyde in the gut before drinking. The mechanism is biologically interesting because acetaldehyde is one contributor to alcohol-related symptoms.

However, hangovers are caused by many overlapping processes. Even if a product affects acetaldehyde in the gut, it does not reverse dehydration, poor sleep, stomach irritation, inflammation, impaired judgment, or the liver’s limited ability to metabolize alcohol. Independent clinical evidence proving meaningful hangover prevention remains limited.

Why Some Drinks Cause Worse Hangovers

Total alcohol dose matters most, but drink type can also matter. Darker alcohols such as bourbon, whiskey, brandy, and red wine often contain more congeners than clearer spirits such as vodka. Congeners are compounds produced during fermentation and aging that may worsen hangover severity in some people.

Sugary drinks can also worsen symptoms for some patients by increasing total alcohol intake, causing blood sugar swings, or irritating the stomach. Carbonated drinks may speed alcohol absorption in some settings. Mixing multiple drink types often matters less than the total amount consumed, how quickly it was consumed, whether the person ate, and how close to bedtime drinking occurred.

A Practical Plan for a Lower-Risk Night Out

The best hangover strategy starts before the first drink. Decide ahead of time how many drinks you plan to have, what time you want to stop, and how you will get home safely. A standard drink is smaller than many people realize: about 12 ounces of regular beer, 5 ounces of wine, or 1.5 ounces of 80-proof liquor. Many cocktails, heavy pours, tall beers, and high-alcohol craft beers count as more than one drink.

Before drinking, eat a real meal and start hydrated. A practical starting point is 16 to 24 ounces of water or an electrolyte beverage before alcohol. Food that includes protein, carbohydrates, fat, and salt can slow alcohol absorption and reduce stomach irritation. For patients prone to reflux or sour stomach, one to two calcium carbonate tablets, such as Tums, may help with acid symptoms.

During the night, pace matters. A useful rule is to have one water or electrolyte beverage for every two alcoholic drinks, and to drink alcohol slowly rather than taking shots or drinking rapidly. Alternating with water does not make alcohol safer in unlimited amounts, but it can reduce dehydration, slow the pace of drinking, and make it easier to stay within the limit set earlier. Avoid mixing alcohol with sleep medications, benzodiazepines, opioids, muscle relaxers, or other sedating substances.

Before bed, focus on safe recovery rather than trying to “cancel out” alcohol. Hydrate with an electrolyte beverage if tolerated, but do not force excessive water. A multivitamin may help replace some nutrients over time, especially B vitamins. One to two 325 mg aspirin tablets, one to two calcium carbonate antacid tablets, B12 injections, and oral or injectable ondansetron (Zofran) may be helpful for some patients.

The final step is sleep. Alcohol disrupts sleep quality, so stopping earlier, hydrating, avoiding late-night heavy meals, and allowing enough time in bed are more useful than most “hangover hacks.” If someone has repeated vomiting, confusion, slow breathing, chest pain, severe abdominal pain, head injury, inability to stay awake, or concern for alcohol poisoning, they need urgent medical care.

Recovery: What to do the Next Day

Recovery should focus on supportive care:

  • Rehydrate with water and electrolytes
  • Eat a balanced meal when tolerated, fattier foods may be helpful
  • Prioritize sleep or a nap if possible
  • Use caffeine cautiously if it does not worsen anxiety, reflux, or dehydration
  • Choose bland foods if nauseated
  • Take a gentle walk if it feels good
  • Avoid intense training if dizzy, dehydrated, nauseated, or sleep-deprived
  • Avoid more alcohol as a “cure”

A light workout may help some people feel better once they are hydrated and eating normally, but intense exercise during a hangover can worsen dehydration, dizziness, injury risk, and poor recovery.

Vitamin and Nutrient Depletion

Alcohol can affect nutrition in several ways. A single night of drinking is unlikely to cause major vitamin depletion by itself, but repeated heavy alcohol use can reduce intake, impair absorption, increase urinary losses, irritate the gut, and interfere with how the liver stores and processes nutrients.

Several nutrients are especially relevant:

  • Thiamine, or vitamin B1: Heavy or chronic alcohol use can deplete thiamine. Severe deficiency can affect the brain and nervous system, which is one reason thiamine is often prioritized in medical care for patients with heavy alcohol use.
  • Folate: Alcohol can impair folate absorption and metabolism. Low folate may contribute to anemia, fatigue, poor recovery, and elevated homocysteine.
  • Vitamin B12: Alcohol-related gastritis, poor diet quality, gut inflammation, or liver disease can contribute to low or borderline B12 status. Low B12 can worsen fatigue, neuropathy symptoms, mood changes, and brain fog.
  • Magnesium and potassium: Increased urination, vomiting, diarrhea, and poor intake can contribute to electrolyte losses, which may worsen weakness, cramps, palpitations, headache, and fatigue.
  • Vitamin D and zinc: These may be lower in some people with frequent alcohol use, especially when diet quality, sleep, liver health, or gut health are also affected.

For occasional hangovers, the focus should be hydration, electrolytes, food, and sleep. For patients with frequent hangovers, fatigue, neuropathy, anemia, brain fog, elevated liver enzymes, poor recovery, or heavy alcohol intake, it may be worth checking labs such as CBC, CMP, B12, folate, magnesium, vitamin D, iron studies, and other markers based on symptoms. Supplements can help when a deficiency is present, but they should not be used to offset ongoing excessive alcohol intake.

IV Fluids and Clinic-Based Support

IV fluids can help selected patients feel better when dehydration, nausea, vomiting, headache, or poor oral intake are major contributors. They may also allow supportive medications to be given when clinically appropriate.

A medical evaluation may be appropriate if hangover symptoms are severe, recurrent, associated with vomiting or dehydration, or occurring in the setting of other health concerns such as diabetes, liver disease, kidney disease, heart disease, medication interactions, or possible alcohol withdrawal.

When a Hangover May be a Warning Sign

A single hangover may simply reflect a night of drinking more than intended. Frequent hangovers are different. They can be a sign that alcohol is affecting health, sleep, relationships, performance, or decision-making.

Warning signs include blackouts, injuries, drinking more than planned, needing alcohol to relax or sleep, hiding alcohol use, missed responsibilities, worsening mood or anxiety, elevated blood pressure, abnormal liver enzymes, reflux, weight gain, poor sleep, reduced training recovery, or needing alcohol the next day to feel normal.

These patterns deserve a conversation with a clinician. Early support is easier than waiting until alcohol causes major health, relationship, legal, or work problems.

When to Seek Urgent Care

Seek urgent medical care for:

  • Severe confusion
  • Repeated vomiting
  • Seizures
  • Slow, irregular, or shallow breathing
  • Blue or pale skin
  • Low body temperature
  • Inability to stay awake
  • Chest pain
  • Severe abdominal pain
  • Head injury
  • Fainting
  • Severe dehydration
  • Concern for alcohol poisoning
  • Withdrawal symptoms after stopping alcohol

Alcohol poisoning and alcohol withdrawal can both be life-threatening.

How Much Alcohol is Too Much?

Alcohol affects the brain, liver, heart, blood pressure, sleep, mood, metabolism, gut, immune system, and injury risk. Even when alcohol use feels socially normal, it can still exceed medical risk thresholds.

In the United States, one standard drink contains about 14 grams of pure alcohol. This is roughly equal to 12 ounces of regular beer, 5 ounces of wine, or 1.5 ounces of 80-proof distilled spirits. Many cocktails, large pours, craft beers, and mixed drinks contain more than one standard drink.

Binge drinking is typically defined as a pattern that brings blood alcohol concentration to 0.08% or higher. This often occurs with 5 or more drinks for men or 4 or more drinks for women in about 2 hours. Heavy drinking is generally defined as 5 or more drinks on any day or 15 or more drinks per week for men, or 4 or more drinks on any day or 8 or more drinks per week for women.

Alcohol use may be affecting health if a person notices frequent hangovers, blackouts, drinking more than intended, difficulty cutting back, needing alcohol to relax or sleep, missed responsibilities, relationship conflict, injuries, worsening anxiety or depression, reflux, high blood pressure, abnormal liver enzymes, weight gain, poor sleep, or reduced exercise recovery. Another warning sign is using alcohol to feel normal. Morning drinking, withdrawal symptoms, tremors, sweating, anxiety, nausea, or feeling unable to stop after starting should be discussed with a clinician.

Alcohol-related health concerns are common, and help should be practical and nonjudgmental. A primary care clinician, mental health professional, addiction medicine clinician, or trusted medical provider can help assess risk and build a plan.

Support may include counseling, therapy, support groups, treatment of anxiety or depression, sleep evaluation, medications for alcohol use disorder, or supervised detox when needed. Patients who drink heavily every day should not suddenly stop without medical guidance because alcohol withdrawal can be dangerous.

Urgent medical care is needed for severe confusion, repeated vomiting, seizures, slow or irregular breathing, low body temperature, inability to stay awake, chest pain, severe abdominal pain, head injury, or concern for alcohol poisoning.

A Better Way to Recover and Prevent the Next Hangover

The best strategy is prevention: drink less, drink more slowly, eat beforehand, hydrate, avoid late-night drinking, and avoid mixing alcohol with sedating medications or other substances. When a hangover happens, recovery should focus on fluids, electrolytes, food, sleep, stomach support, and safe medication choices.
